Middle & High School Faith Formation Registration Info

We are now accepting registrations for Middle School and High School (Confirmation) Faith Formation. Registration will take place in person on designated dates in May.

Required Documents (Mandatory):

  • Baptismal Certificate
  • First Communion Certificate
  • Birth Certificate

High School Confirmation Registration Dates:

  • May 13 | 9:00am – 5:00pm
  • May 14 | 12:00pm – 7:00pm
  • May 17 | 12:00pm – 5:00pm

Middle School Registration Dates:

  • May 20 | 9:00am – 5:00pm
  • May 21 | 12:00pm – 7:00pm
  • May 24 | 12:00pm – 5:00pm

Class Fees:

  • Middle School: $150
  • High School (Confirmation): $250

Please scan the QR code on the flyer to access registration forms ahead of time. We encourage families to complete forms in advance to help make the process quicker.
